National Health Insurance (国民健康保険, kokumin kenkou hoken)

National Health Insurance (NHI) and How it Works

National Health Insurance (abbreviated in Japanese as 国保, kokuho) is a medical insurance system designed for all people living in Japan (including students, self-employed and part-time workers) who are not enrolled in any type of health insurance through their work provider.

If you are NOT already enrolled in health insurance in Japan or are not receiving Livelihood Protection, you must enroll in National Health Insurance. Please complete all enrollment/withdrawal paperwork within 14 days, such as after moving to Kasama City, changing jobs, etc. (See table below.)

*From March 2021 you can use your "My Number" Card as a Health Insurance Card

! You need the following documents for ALL procedures regarding the NHI.

Please have them with you:

1. ID: Residence Card (在留カード, zairyu kādo) or Special Permanent Resident Card (特別永住者証明書, tokubetsu eijuusha shoumeisho)

2. Individual Number¹: My Number Card (個人番号カード, kojin bango kādo) or My Number Notification Card (通知カード, tsuchi kādo)
¹ If you do not have your Individual Number yet, please apply for it at the Citizens Affairs Division (市民課, shimin ka) or Kasama / Iwama Branch Citizen Service Division (市民窓口課, shimin madoguchi ka).

 ² The most common "another insurance plan" is Employees' Social Insurance. In Japan, Social Insurance (社会保険, shakai hoken) combines Health Insurance (健康保険, kenko hoken) and Pension Insurance (厚生年金, kosei nenkin) into one system.

Once all appropriate documents are submitted, you will be reimbursed based on the most logical and cost-effective route/means of transfer.

Childbirth

When you have given birth to a child

Funeral Expenses

When a loved one has passed away

National Health Insurance Tax (国民健康保険税, kokumin kenko hoken zei)

National Health Insurance Tax is calculated as follows. In the case of an irregular enrollment/withdrawal, calculations will be broken down by month.

1 Income percentage is calculated based on your income from the previous year.
2 The per capita rate is the amount that each enrolled person pays.

Tax Payments

In Japan, the head of the household (世帯主, setainushi) is responsible for paying the National Health Insurance Tax, even if that person is enrolled in a separate insurance plan. Therefore, all invoices will be mailed to the head of the household. However, taxes will only be calculated based on household members who are actually enrolled in the National Health Insurance (NHI) plan.

National Health Insurance Tax Reduction System

Under the Legal Tax Reduction System (法定軽減, houtei keigen), an NHI member may be exempted from per capita amount, depending on the income of the insured person or the head of the household for the previous year.

Applications are not required since the reductions apply automatically to illegible persons.

Tax Relief

If in the case of some sort of accident or natural disaster paying your taxes becomes difficult, please consult the corresponding division at the City Hall as soon as possible.

Don’t forget to file your taxes!

National Health Insurance Tax is based on your income from the previous year. Make sure that everyone in your household files their taxes every year, as this will affect how much money you are able to receive for medical expenses, etc., and may result in you paying more than usual.
